Registration Requirements for checking out material at the Douglass Public Library, 319 S. Forrest-

To obtain library cards at Douglass Public Library, you need to fill out a patron card and show proof of address.

Children below the ages of 14 must have parents fill out and sign the registration card.

Video checkout for children below the ages of 14, requires a guardian's signature on the application card.

Internet usage requires a patron to be in good standing and 18 years of age, anyone under 18 will need parental approval.
